If you wish to close your pharmacy permanently, follow these step by step instructions:
Step 1: Submit a Letter of Intent
A letter of intent (click here for a sample ) must be received by Client Services within 7 days of closing the pharmacy.
You may submit your application by fax, email or regular mail.

Step 2: Confirmation from OCP
Once your letter of intent has been reviewed, Client Services will respond by email confirming receipt and will include a copy of the Pharmacy Closing Statement to be submitted after the closing date.
Step 3: Closing Day
On closing day, Client Services will contact the pharmacy to confirm whether the pharmacy has officially closed to the public. Once confirmed, the pharmacy’s status will be changed from Active to Closed and will no longer appear on the College’s Public Register.
Step 4: Submit the Pharmacy Closing Statement
A completed Pharmacy Closing Statement must be returned to Client Services within 30 days of the closing of the pharmacy.
